1. Photograph or screen capture the receipt provided at the time of vaccination. You can also obtain your vaccination receipts from Ontarios Vaccination webpage
3. Follow the COVID-19 link
4. From the COVID-19 menu select the Vaccine Passport
5. Select your affiliation with the College.
6. Employee/Student sign in using your Algonquin College credentials. Contractor/Visitor register by entering in your information.
7. From the Start Application menu select Begin Application.
8. From the Apply for Vaccine Passport menu select your vaccine passport application.
9. From the Confirm Your Details page enter your phone number and your affiliation .
10. Enter your vaccination information into the fields. Photos are the only acceptable file to upload.
11. Click Submit
For Conscience Or Religious Belief
For your child to be exempted due to conscience or religious belief, you must contact your local public health unit:
- Let your local public health unit know you wish to have your child exempted from the required vaccines.
- The PHU will:
- advise you on the steps to take in completing a valid exemption, including watching the vaccine education video.
Once its complete, you must get it signed by a commissioner for taking affidavits in Ontario.
Make copies of your:
- signed Statement of Conscience or Religious Belief form
You will need to submit the original versions.
It is important that you keep your copies because the ministry and local public health units do not keep records of your exemption documents.

CHICAGO Gov. JB Pritzkers administration dropped a statewide COVID-19 vaccine mandate for college students and faculty members last week, leaving local universities to adopt their own policies for vaccine requirements.
Pritzker also eliminated requirements for emergency medical service providers to be vaccinated, while upholding mandates for staff at K-12 schools, day cares, congregate care facilities and other health care facilities.
I continue to urge all Illinoisans to make sure theyre up to date on their COVID-19 vaccine to ensure the most at-risk populations and those unable to be vaccinated are protected from the serious side effects of this disease, Pritzker said in a news release.
Block Club reached out to more than a dozen colleges and universities about whether vaccines will be required at their campuses. Heres what they said.

Utah College And Vocational Institution Immunization Recommendations
It is recommended that all students be appropriately immunized against measles, mumps, rubella, human papillomavirus, meningococcal disease, hepatitis B, tetanus, diphtheria, pertussis, influenza, varicella, polio, and hepatitis A, in accordance with the recommendations of the Centers for Disease Control and Prevention and the Advisory Committee on Immunization Practices as a prerequisite to matriculation or registration to a Utah college, university or other post-high school educational or vocational institution.
Do College Students Need Mmr
In the U.S., almost everyone receives the CDC-recommended two doses of MMR, the vaccine against measles, mumps, and rubella, as young children. The MMR affords good protection against these diseases, and routine vaccination with a third dose of MMR isnt necessary for all students.
Still, there are some circumstances in which youll want your student to get a dose of MMR before he or she arrives on campus. If for some reason your child didnt receive MMR vaccination at a young age, they should have two doses, separated by 28 days.
Heres why: Although most recent measles outbreaks havent been on college campuses, they are considered high-risk settings. Mumps is sometimes a concern for college students as well. Between 2015 and 2017, two large outbreaks at universities in Iowa and Illinois caused several hundred students to get sick.
The CDC usually recommends that people in communities with an active mumps outbreak receive an additional dose of MMR. A 2017 study in the New England Journal of Medicine found that a mumps booster in an outbreak significantly reduced the risk of infection. For measles outbreaks, the CDC recommends people who arent adequately vaccinated catch up, but people generally dont need an extra dose of MMR if they’re already immune to measles.

A still-expanding group of U.S. colleges and universities say students must receive a COVID-19 vaccine before arriving on campus. The first institutions to announce the mandate were private, with Cornell University and Duke University leading the way. More and more universities, private and public, have followed suit. Currently, over 1,000 colleges and universities require the COVID-19 vaccine for residential students.
Most colleges already require on-campus students to be vaccinated against viral diseases like measles, mumps, and rubella. Government agencies suggest that schools may similarly require the COVID-19 shots.
Some state lawmakers and at least two lawsuits argued that inoculation can’t be made mandatory while the experimental vaccines retained their Emergency Use Authorization status, but full FDA approval of the mRNA-based Pfizer-BioNTech vaccine on August 23 has already triggered a surge of college COVID-19 vaccine mandates.
What Are The Top Vaccines That College Students Need

Which Colleges Will Require Vaccines
Theres an interesting breakdown of how schools are navigating this field. For example, Rutgers, Brown, Cornell and Northeastern told students they need to get vaccinated, per AP.
But Virginia Tech told students that they cant legally require the vaccine because the U.S. Food and Drug Administration has only allowed the emergency use of the vaccines and hasnt given them its full approval, according to the AP.
What Is The Policy/process Around Rapid Testing
As detailed in the Colleges Mandatory COVID-19 Vaccination Policy Addendum , alternate safety protocols are being administered for eligible College Community Members who have not achieved a full COVID-19 Vaccination status and are required to attend on-campus or in-person activities.
Negative results from Rapid Antigen Tests are being relied upon to provide a high level of assurance that a College Community Member is not carrying the COVID-19 virus if the Member is not fully vaccinated.
To support this requirement, Algonquin College is beginning to offer Rapid Antigen Tests to eligible College Community Members starting Thursday, September 9th.
The tests are being offered at no cost to College Community Members who are required to access campus AND have received approval for a bona fide exemption. The test being offered is the Panbio Rapid Antigen Test Nasopharyngeal swab.
Effective September 13th, the policy requirement of 2 negative rapid antigen tests from 2 different days during the previous seven days will be applied.

The Ohio Revised Code Section 1713.55 states that beginning with the academic year that commences on or after July 1, 2005, an institution of higher education shall not permit a student to reside in on-campus housing unless the student discloses whether the student has been vaccinated against meningococcal disease and hepatitis B by submitting a meningitis and hepatitis B vaccination status statement.
ORC Section 3701.133 states that the Ohio Department of Health shall make available on its Web site information about meningitis and hepatitis B, the risks associated with the diseases and the availability and effectiveness of the vaccines. ODH shall also make available, on its Web site, in a format suitable for downloading, a meningitis and hepatitis B vaccination status statement form that complies with the guidelines outlined in ORC Section 3701.133, .

Covid Vaccine On Campus
During the 2020 COVID-19 outbreak many colleges moved to online college classes. As states reopen, universities are welcoming students back on campus. Some schools require that students receive the vaccine before resuming regular classes. Students should research their current school policies on the COVID vaccine.
Currently, there are three different vaccines available. The Pfizer-BioNTech and Moderna both require two doses to be considered fully vaccinated. The Johnson & Johnson’s Janssen vaccine has resumed in the United States and requires one dose. Below are three vaccine finders that allow students to locate vaccination sites. Students should also look for their state specific vaccine finder.
What If I Don’t Want To/can’t Get Vaccinated
As mentioned above, there are both medical and non-medical exemptions for students who cannot or do not want to get vaccinated. Justin Sloan, the VP of Institutional Effectiveness and Planning at St. Edward’s University said it is very rare for a student to not meet the exemption process if that is their decision, whether it be for medical, religious, or personal reasons.
Every school will have different protocol for the exempt students, but for the most part, they will still be able to attend classes and live on campus normally. However, Justin warns that, at least in the case of St. Edward’s, while a non-vaccinated students will be allowed on campus, there may be some external programs they cannot participate in, like study abroad.
“We’re not telling the student they can’t learn with us,” he said. “We’re not telling them they can’t live on campus, but we are being transparent that there are some activities that are provided by third parties that may not be available to them.”
