How To Obtain/send A Copy Of Your Slc Transcript

Complete the Transcript Request Form and submit it in person, by email or to the appropriate campus fax number.

The request form can be either printed out and completed or by fill-able PDF however, you must print and sign the bottom – Electronic signatures are not accepted. A signature is required to obtain/send your transcript to your destination.

Note: BScN and BBA graduates are advised to order transcripts directly from Laurentian University, for proof of completion and admission purposes.

Hold Transcript For Grades Or Degree

A request may be submitted to delay printing the transcript until after grades are posted or degrees for the current semester have been recorded on the transcript. However, if you are expecting a grade change, do not order your transcript until you are sure the grade has been changed. The Registrar will not hold transcripts for maintenance or grade changes.

Transcript orders held for degrees are released approximately four weeks after your conferral date. If the status of your degree in Academic History of Self Service indicates that the degree has been Awarded and is no longer Pending prior to the release date listed on the Academic Calendar, you may call the Transcripts Unit at 217-333-9778 and request that the Hold for Degree be removed and your transcript order processed.

Students applying to transfer out of the University to an institution that uses the Common Application, should verify first if the destination institution requires the College Report for transfers. Identity Verification & Uwnetid

Parchment requires student identity verification and transcript order authorization to comply with FERPA regulations. Students may authenticate their identity by being logged in with their UWNetID which allows for quicker verification by Parchment. However, a UWNetID is not required to place a transcript order. Students will be asked to provide personal identification markers that Parchment will use to match with UW student identity records.

Official transcripts display the same name that is associated with your student record in the Universitys student database. Please keep this in mind when entering your information on the ordering screen and be sure to include any previous names if you have changed your name since attending UW. Be advised that if you have a different name on file with the Alumni Association or another UW organization, or if you are a current or previous employee , the name that displays on your MyUW and the transcript ordering page may be different than what is in the student database. The same name that appears on your unofficial transcript on MyUW is what will appear on an official transcript.

Part of OTC admissions is to submit official high school or high school equivalency and official college transcripts.

• First time students may schedule STAR and register for their first semester of classes without official high school or high school equivalency transcripts on file. However, OTC requires official transcripts to be eligible for potential financial aid disbursement.

• Transfer and re-admitted students must have all official college transcripts on file prior to registering for classes their 2nd semester at OTC. The college must receive official transcripts and evaluate credits for transfer credit in order to meet prerequisite requirements.
